At 22 Plastic Surgery, we reserve surgical dates exclusively for our patients and arrange operating room time, staff, and resources well in advance. For this reason, the following cancellation policies apply: How to Cancel: All cancellation or rescheduling requests must be submitted in writing via email to info@22plasticsurgery.com. The date and time the email is received will determine applicable fees.Cancellation Timeframes & Fees: Cancellations within 14 days of scheduled surgery: 25% of the total surgical fee will be withheld. A new $500 deposit will be required to reschedule surgery. Cancellations within 7 days of scheduled surgery: 50% of the total surgical fee will be withheld. A new $500 deposit will be required to reschedule surgery. Same day cancellations: 100% of the total surgical fee will be withheld. A new $500 deposit will be required to reschedule surgery. Additional Policy Terms: Deposits and forfeited fees are non-refundable. Fees withheld due to cancellation may not be applied to future procedures unless explicitly approved by the practice. Failure to appear for surgery without proper notice will be treated as a same-day cancellation. Medical exceptions, if any, are considered on a case-by-case basis and require documentation. By scheduling surgery with 22 Plastic Surgery, patients acknowledge and agree to this cancellation policy.

Gift cards are available for purchase at our practice and may redeemed for non- surgical and surgical services, as well as approved products, unless otherwise specified at the time of purchase. Gift cards cannot be redeemed for cash, credit, or refunds. All gift card purchases are final and non-refundable. Gift cards are non-transferable and may only be used by the recipient listed at the time of redemption, unless otherwise approved by the practice. Gift cards do not expire or expire after a specified period (commonly 12 months), in accordance with applicable state laws. Promotional discounts or special offers may not be combined with gift card redemptions unless expressly stated. For surgical procedures, gift cards may be applied toward the total cost but do not replace required deposits unless specifically approved. Standard cancellation and rescheduling policies apply. Gift cards are subject to applicable federal and state laws. The practice reserves the right to modify this policy at any time.

These Product Sales Terms & Conditions apply to all retail products sold by 22 Plastic Surgery, including but not limited to medical-grade skincare, cosmeceuticals, supplements, post-procedure care products, and aesthetic devices. Due to health, safety, and hygiene regulations, all product sales are final. We do not accept returns or refunds for opened, used, or tampered products. Unopened, unused products in their original sealed packaging may be eligible for exchange within 14 days of purchase, at the discretion of 22 Plastic Surgery. A valid receipt is required. No cash or credit card refunds will be issued. If a product is defective or damaged at the time of purchase, please notify us within 7 days of purchase. Upon review, we may offer a replacement or store credit. Manufacturer defects are subject to the manufacturer's policies. Product recommendations are provided by licensed medical professionals or trained staff based on general patient information. Individual results may vary, and product use does not replace medical evaluation or treatment unless specifically advised by a licensed provider at 22 Plastic Surgery. Prices are subject to change without notice. Product availability is not guaranteed, and 22 Plastic Surgery reserves the right to limit quantities or discontinue products at any time. Promotions, discounts, and bundled pricing may not be combined unless explicitly stated. Promotional sales are final and not eligible for price adjustments. Payment is due in full at the time of purchase. We accept approved forms of payment including cash, credit cards, debit cards, and approved financing options. By completing a purchase, the patient agrees to contact 22 Plastic Surgery directly to address any concerns prior to initiating a chargeback or payment dispute. These terms are governed by the laws of the state in which 22 Plastic Surgery operates. To ensure compliance with Utah state law and protect patient safety, 22 Plastic Surgery enforces the following age-related policies for elective and cosmetic procedures. Under Utah law, a person must be 18 years of age or older to be legally considered an adult for the purposes of consenting to elective medical or cosmetic procedures. A minor under the age of 18 may not undergo elective cosmetic or aesthetic surgical procedures at 22 Plastic Surgery without proper parental or legal guardian consent. This includes, but is not limited to: Cosmetic surgery (e.g., rhinoplasty, breast augmentation, liposuction), Non-surgical aesthetic treatments that require medical evaluation. For patients under 18: A parent or legal guardian must be present, and sign all required consent and authorization forms before any elective or cosmetic treatment is provided. The consenting adult must provide valid government-issued identification and proof of legal status as the parent or guardian. No elective surgical procedures may be scheduled or performed without completed and signed consent documentation from a parent or legal guardian. Per Utah law, individuals under 18 who are: Legally emancipated, in active military service, or married, may be considered able to consent to their own healthcare decisions. Validation of legal status (such as an emancipation order) must be submitted and verified prior to treatment. 22 Plastic Surgery reserves the right to decline services to any minor for whom it is not medically appropriate, even with parental or guardian consent. This may include procedures that are contraindicated due to developmental status, medical history, or other clinical considerations. By scheduling or receiving services at 22 Plastic Surgery, patients acknowledge understanding and agree to abide by the age restrictions and consent requirements set forth above.
